We can deliver annual health assessments for your Home Care Package clients. We understand the challenges faced by many Home Care Package providers and we are here to support you where you may have gaps in resourcing or expertise.
Our annual health assessment service is designed to support Home Care Package organisations with the ongoing assessment and planning requirements of Standard 2. Our assessors are experienced Registered Nurses, and we service all Melbourne areas.

Our nurse-led health assessments
We’re here to support you in delivering exceptional care and maintaining the highest standards of compliance. We offer both initial health assessments for your new clients and annual assessments for all existing clients. The assessment typically takes 1-2 hours (depending on the client) plus write up.
Obtain comprehensive client health histories & functional assessment information
Registered Nurses conduct detailed assessments that enable HCP Case Managers to create tailored care plans and service schedules for each client.
Identify additional subsidy eligibility
Our Assessments can identify if your client qualifies for additional subsidies or the dementia supplement through a Cognitive Decline Assessment.
Manage and mitigate risks
We develop individualised risk management plans, identifying potential risks and recommending effective mitigation strategies.
Apply for higher funded packages
Our Assessments can help identify whether your client may be eligible for any additional subsidies to their Package funding. We can also carry out a Cognitive Decline Assessment which can be used to apply for the dementia supplement, if indicated.

Conducting regular Health Assessments for your clients will help you to comply with Standard 2 of the Aged Care Quality Standards.
