How we help

Whether your reablement goals include getting the house clean or picking up groceries and cooking, we can help. We want to help you remain comfortable while you recover, and get back on your feet.

Clinical supports

Ensuring you receive the right clinical care is one of the most important factors when recovering from an injury or illness. Our Community Nursing team is made up of registered nurses who can provide you with follow up clinical care in the home after discharge from hospital. Short-term supports include:

One size does not fit all when it comes to finding the right support worker for you. You tell us your preferences, and we’ll find the right person to provide your short-term supports. We take into account preferences and needs regarding:

Day-to-day activities

While you are recovering from an acute injury or illness, there may be limitations on which everyday activities you can participate in. We can assist with household tasks, personal care, such as:

Assistance with post acute care costs

You may be eligible for short term supports through hospital programs. You can find out more through the hospital you have been discharged from. 

Alternately, depending on the nature of your accident, you may be eligible to make a claim through the Transport Accident Commission.
