We support older people live safely and independently in their own homes through personalised in-home aged care, guided by clinical oversight.
Absolute Care & Health is a registered Support at Home provider based in Melbourne, delivering government-funded in-home care services across metropolitan Melbourne.
Under the Support at Home program, every provider charges the same management fee but the level of care you receive can vary greatly.
With Absolute Care & Health, your 10% care management fee includes clinical oversight by Registered Nurses.
Unlike most providers who assign non-clinical care managers, our Care Partners are Registered Nurses. They actively monitor your health, identify early warning signs and adjust your care before small issues become serious.
The result? Safer care, earlier intervention and a care plan that evolves with your needs.
All for the same 10% fee — with greater peace of mind.
With Registered Nurses anchoring our care management in a strong clinical framework, you can feel confident that your safety, health and wellbeing always comes first.
Strong clinical oversight of your care gives you nurse-led care planning, expertise in identifying and responding to changing needs, clinical advice and a truly person-centered approach.
Our Registered Nurses bring medical knowledge and clinical judgment to your care. They understand the signs and symptoms that others might overlook, ensuring your care is guided by professional insight — not guesswork.
Because our Registered Nurses are clinically trained, small changes in your condition are recognised early. That means potential issues can be addressed before they become serious, helping prevent unnecessary hospital visits.
With professional oversight and proactive care adjustments, clients maintain independence for longer, experience fewer health setbacks and enjoy a higher quality of life at home.

Switching Support at Home provider is easier than you might think and there are many benefits that could be realised. We’re here to make things simple for you with our 6 steps to switching guide:
Some care providers have minimum notice periods when you change providers. Check your Home Care Agreement or ask your current provider to find out if these apply to you.
Write to your current provider and tell them that you are moving your Support at Home and giving them notice as specified in your existing Home Care Agreement.
Ask your current provider to confirm in writing the last date that they will provide your care – this is known as your cessation date.
Call My Aged Care to tell them you’re switching your Support at Home to a new provider. Ask for your new Unique Referral Code.
Give your new provider your Unique Referral Code so they can access your My Aged Care records and register as your new care provider.
Any unspent funds from your package will be transferred to your new provider.
“Fantastic service and they really do care about their clients health and well-being. Couldn’t be happier knowing my Grandmother is well taken care of with Absolute Care & Health.
The communication between my Grandmother’s Case Manager, herself and me is second to none! Whenever I need a question answered or my Grandmother requires anything to assist with her health and quality of life is promptly organised and delivered.
This makes her life less stressful and makes me feel confident she is taken care of and in good hands. A special Thank you to Nancy, her Case Manager for the Fantastic work she does managing all my Grandmother’s special needs.”
Seth, Client’s Grandson

As a Support at Home recipient, we are required to give you a Home Care Agreement which outlines our obligation to provide your Support at Home services.
You are free to cancel the Care Agreement and leave us whenever you want by providing us with 14 days’ notice.
We do not charge any Exit Fees if you choose to leave us and we will support you to transition to a new provider.
Absolute Care & Health directly employs our own staff and ensures they a held to a high standard of quality and consistency.
We make significant investments in continuous training and development to ensure our in-house carers and nurses consistently surpass industry standards.
We have a minimum of a 2-hour personal care visit and a 30-minute minimum for nursing services.
You will have a dedicated Care Partner who is there for you. Their job is to make sure that your Support at Home care plan is working for you and that you are getting the support and services that you need.
They are responsible for developing your Care Plan with you and ensuring it is appropriate to meet your goals within your budget. They will organise the services and products you require as part of your budget and if you receive Care services from us, they will ensure that the right Carer is selected for you.
Your Care Partner will stay in touch with you regularly to make sure the supports you are receiving remain appropriate to your needs and situation and that your Care Plan is reviewed and modified should your needs change.
Once you have decided you would like to receive care with Absolute Care & Health, we will generally book in your care management assessment and begin your services within 7 days.
While we strive to provide you with a consistent carer so that you can develop a friendly relationship with them, we do create a team of carers that can assist you in case of carer sickness or leave arrangements.
We do this so you are familiar and comfortable with any backup carers that may need to step in and provide your service.
Your Support at Home funding cannot be taken away from you because you switch providers. It will remain with you no matter how many times you change providers.
However, you must start with your new provider within 56 days of finishing your care with your old provider. If you need longer to decide on a new provider, you can ask for an extra 28 days.
Your Home Care Agreement with your current provider will specify how much notice you must give them. You are not obliged to give any more notice than is specified here. If you have a long notice period, they may agree to agree to shorten it if you ask.
They are not allowed to do this. The Government has ruled that any unspent funds will move with you. The only money your current provider can keep is the cost of any services or products they have already provided to you that is specified in your Home Care Agreement with them.
